Medical examination is a complex of diagnostic tests that is carried out for the purpose of preventing and evaluating the health of Americans. Based on the results of a comprehensive study, each patient is assigned a health group and, if necessary, is registered in a dispensary.
Conducting medical examinations is regulated by the following regulations:
- By government decree No. 1391-R of 27.06.19.
- By order of the Ministry of health No. 124-n dated 13.03.19.
The order of the Ministry of health came into force on may 6, 2019. It determines the procedure for conducting medical examinations, the years of birth of patients who fall under the passage in 2021-2022, and other important nuances.
Previously, all citizens of the American Federation starting from the age of 18 could be examined every 3 years. After the Ministerial order, the frequency of its implementation for different age groups has changed.
- Persons aged 18-39 years – once every 3 years.
- 40 years and older – annually.
For the 40+ age category, medical examinations are performed annually in 2021 and beyond. There is a separate category of citizens who are also examined every year. This:
- disabled combat veterans;
- participants and disabled people of the second world war;
- residents of besieged Leningrad who received disabilities due to General illness and other reasons;
- former prisoners of concentration camps, Nazi ghettos and other similar places who received disabilities;
- working citizens who are 5 years away from their old-age pension;
- working pensioners, including those who have taken a well-deserved rest due to long service.
Citizens born in 1930-1981 undergo a routine medical examination every year.
Every 3 years
For those who are not yet 40 years old, there are defined such years of birth that fall under the mandatory passage.: 2003, 2000, 1997, 1994, 1991, 1988, 1985, 1982.
The easiest way to determine whether you need to take a survey in a particular year is to divide the age by 3. if the division results in a number without a remainder, then you need to.
Table 1. What years will pass medical examinations in 2021
|Years of birth||Age group|
The calculations take into account the year, not the date of birth. If a person was born on December 29, 1993, they can undergo medical examinations in may, July, October, or any other month until December 31, 2021.
Monitoring of children and adolescents
For children and adolescents, the Ministry of health has established its own schedule of visits to medical institutions and routine preventive examinations.
Table 2. Clinical examination of children and adolescents in 2021
|Age group||The frequency|
|0-12 months||monthly at the pediatrician’s office|
|1-2 years old||once every 3 months|
|2-3 years old||once every 6 months|
|from the age of 4||every year|
An extended medical examination (pediatrician and specialists of a narrow profile, necessary laboratory tests) should be carried out at the age of:
- 3 years old – when entering a preschool institution;
- 6-7 years – before entering a General education institution;
- 14-17 years is the so-called puberty period.
The basic nuances
Medical examinations in New-York and other regions of America are included in the list of compulsory health insurance services and are carried out free of charge. You can pass it in any medical institution that has a license. Usually, citizens apply to the polyclinic at the place of residence for which they are assigned.
The organization of a preventive examination is carried out by a district therapist serving a specific territory. Its responsibilities include::
- informing citizens about the polyclinic’s work schedule;
- examination of the patient;
- referral for additional examination if necessary;
- determination of a health group based on survey results;
- registration at a dispensary if necessary.
Stages of medical examination
Medical examination consists of two stages:
- Stage 1-General, conducted in a medical facility;
- Stage 2 – additional studies to clarify the diagnosis.
Table 3. Study of the 1st stage
|Type of research||Age group||Notes|
|UAC expanded + leukocyte formula||From the age of 39||1 time in 6 years|
|ECG||All||At repeated medical examination, women-from 45 years old, men – from 35 years old|
|DOG||Men over 50 years old|
|The fecal occult blood test||From the age of 45|
|Fluorography||All||It is not performed if the patient has already been tested in the current year|
|Cytology of the cervix||Women over 30 years of age|
|Fundus examination, determination of eye pressure||From the age of 39|
|Mammography||Women from 39 years of age|
|Neurologist’s consultation||Since the age of 51||Once every 6 years|
Table 4. Studies of the 2nd stage
|Type of research||Age group||Symptoms|
|Glycated hemoglobin||All||Blood sugar is higher than normal|
|Lipid profile||All||High cholesterol|
|EFGS||All||If you suspect neoplasms in the gastrointestinal tract|
|Duplex scanning of the arteries||Men-from 45 years old; women – from 55 years old||In the presence of risk factors (blood pressure, obesity), disorders of cerebral circulation|
|Consultations of narrow specialists||All||In case of violations detected during the examination or based on the results of tests|
General and biochemical blood tests, ultrasound of the abdominal cavity are not performed during the medical examination – they are prescribed only if the patient has health complaints.
Specialized specialists include a gynecologist, surgeon, proctologist, gastroenterologist, cardiologist, ophthalmologist, and other doctors.
A health group is a conditional characteristic of a patient’s health status. In the order of the Ministry of health No. 124-n, the following classification of groups is introduced::
- the first;
- the second one;
- third (there are two varieties of the third group – “A” and “B”).
Table 5. Group health
|1||The person is healthy|
|2||Chronic diseases are not diagnosed, but risk factors are present||Key risk factors: high blood pressure, blood sugar, high cholesterol, the presence of alcohol or drugs in the blood, obesity|
|3A||Diagnosed with chronic diseases||The patient needs constant medical supervision|
|3b||Chronic diseases are not identified – in the acute stage of the pathology||The patient needs medical care in a specialized institution|
Based on the results of research and taking into account the health group, the patient is assigned treatment or dispensary registration. Dispensary registration involves regular visits to the polyclinic (the frequency is determined by the attending physician). Most often, this applies to people with chronic pathologies of the heart, blood vessels, respiratory organs and other vital systems and organs.
